At KIND PT & Wellness we want to maximize your time at home or at work physical therapy appointment. Prior to your first appointment, please click on the applicable form below, fill out, bring with and/or email to calderks@kindptwellness.com.

Do I need a doctor’s prescription to have physical therapy?In the states of Illinois and Florida, you can see a physical therapist for an evaluation and be treated by a physical therapist without a prescription from a physician.

Does KIND PT & Wellness take my insurance?… and why not?KIND PT & Wellness is a cash-based practice, and therefore, I am not contracted with any insurance companies, which is a really good thing for you. I believe that this is a way that I can provide one on one care that is effective and tailored exactly to your needs. I form a direct contract with you, meaning you and I decide on a course of treatment, not your insurance company. It allows significant change within the first 3-5 visits costing you less money and time. If you do have insurance, upon request, KIND can provide you a receipt including the billing and diagnosis codes that you can submit to your insurance for out of network reimbursement. This is called a super bill. The amount of reimbursement will vary with different carriers.

How do I pay?Most forms of payment are accepted (credit/debit, cash, Apple Pay) as well as payment via HSA or FSA.

What does mobile PT mean?KIND will provide services in the comfort of your home and/or work. We come to you!

Can I see you in the clinic?KIND will also provide services on limited days and times in the clinic in Rockford, Illinois. When scheduling your appointment, there will be an option for you to choose a time that fits your needs with the availability of the clinic.

What do I need to do or bring for day 1?If you are being seen at home, at work, or at the clinic, please have appropriate clothing which means clothing easy to move in and remove as needed to fully assess anatomical landmarks including upper shoulders, back, etc. Please have a pair of tennis shoes to perform exercises in. If you have a mobile visit at home or at work, a space for a treatment table and a small area to stand up and move in is ideal to have prepared before your visit. This allows me to provide a comprehensive evaluation or treatment session for you. For any first visit, please email or bring completed forms which you can find at the top of this page.
